Advanced Procedure Forms
To provide the advanced procedures permitted by the law change effective July 1, 2021, licensees shall complete the Proctoring Attestation Form in conjunction with their proctor. Licensees are required to maintain this document in their records.
Any licensee providing these advanced procedures shall report to the Board any adverse outcomes following each procedure. These reports are required within ten (10) days from the date the licensee is aware of the adverse outcome.
Renewal Notices were mailed to the last known address of all licensees on February 2, 2021. All licenses must be renewed by April 1 annually.

Reinstatement and Relicensure Forms
If you failed to renew your license by April 1, you may apply for reinstatement of your Wyoming license between April 2 to June 30 of the year the license expired.
If you have been previously licensed in Wyoming and failed to reinstate your license by June 30 of the year the license expired, you must apply for relicensure.
To request a change in your name or contact information, please submit the completed form and include your fee, if applicable.
To have your Wyoming license verified to another jurisdiction, please submit the completed form and include your fee of $25.
To request replacement license materials, please submit the completed form and include your fee.
